Community Horizons prides itself on connection and coordination with compassion. We believe that excellent communication is key to providing effective support. Our support brokers are trained to communicate clearly and effectively, ensuring that individuals understand all aspects of the self-directed program. They take the time to listen to individuals’ needs and concerns while also providing guidance and information. Our goal is to empower individuals to make informed decisions regarding their services and supports.

MEET THE TEAM

Joyce Challoner

Lead Support Broker

 

Joyce has demonstrated her strong ability to communicate efficiently. Joyce's personable nature allows her to connect with clients, their families, and other healthcare professionals in a way that puts them at ease and fosters trust.

​

Joyce's background in EMS-I/Fire Fighter and her extensive experience as a Program Director and Division Administrator in healthcare have provided her with a wealth of knowledge and skills. This experience has allowed her to gain a deep understanding of the healthcare industry and to build strong community relationships that contribute to successful outcomes.

In addition to her excellent communication skills, Joyce is action oriented. She does not simply wait for problems to arise; instead, she takes initiative and actively seeks out solutions. This proactive approach ensures that issues are addressed in a timely manner and that clients receive the best possible care. Joyce's dedication to her work and her genuine concern for the well-being of others make her a valuable asset to any team.

Angela Dupke
Support Broker

Angela has always shown her caring nature towards others. Whether it is her clients, her coworkers, or the community, Angela always goes the extra mile to ensure that everyone she encounters feels supported and valued. She genuinely cares about the well-being of others and is always willing to lend a helping hand.

Angela is also dedicated to helping others succeed. She is a firm believer in the potential of every individual and works tirelessly to ensure that they have the tools and resources they need to reach their goals. Whether it is advocating for a client's needs, providing guidance to a coworker, or connecting someone with the right resources in the community, Angela is committed to making a positive impact on the lives of others.

Brenda Lunde

Throughout her career, Brenda has shown a passion for helping others and making a difference in people's lives. She genuinely cares about the well-being of her clients and goes above and beyond to ensure they receive the best possible care.

Brenda's journey in the healthcare field began in 2004 when she started working as a Certified Nursing Assistant (CNA). As a CNA, she experienced firsthand the importance of providing compassionate and personalized care to others. She quickly realized that she had found her calling and decided to pursue a career in healthcare.

Over the years, Brenda has worked hard to expand her knowledge and skills. She has taken on various roles in the industry, including a scheduler, Dietary Manager, Director of Social Services, and even an Interim Administrator. Each position has provided her with unique experiences and allowed her to grow both personally and professionally.

Shawna O'Keefe
Support Broker

Shawna's journey in the field of healthcare has spanned over 26 years, and she has dedicated her life to the service of others. With experience as a Certified Nursing Assistant (CNA) and a med passer, Shawna has been actively involved in direct patient care. However, her passion for helping those with special needs led her to run an adult family home for individuals with special needs, highlighting her commitment to making a difference in their lives.

Moreover, Shawna's talents and expertise extend beyond just caregiving. She has also worked as an assistant director for a memory care unit, showcasing her versatility and ability to handle a diverse range of responsibilities within the healthcare industry. What truly sets Shawna apart is her unwavering dedication to the people she serves. Her compassionate nature and genuine concern for others make her a reliable and trustworthy individual. Shawna takes pride in being the person others can count on, and this commitment is evident in every aspect of her work. 


Outside of her professional life, Shawna enjoys spending time with her loved ones. She has two grown sons and five grandchildren, who undoubtedly hold a special place in her heart. One of her grandchildren, a non-verbal autistic child, has a particularly close bond with Shawna. She finds joy in being there for him and providing him with the love and support he needs.

Pam Passehl

She is hardworking, caring, compassionate, and a strong advocate for ensuring that members' goals are met. With over two decades of experience in the healthcare field, Pam has built strong and long-lasting relationships with members, care teams, guardians, and family members

 

Pam genuinely cares about the well-being of the clients she works with and goes out of her way to ensure that they are comfortable and supported. Whether it's ensuring that they have access to the right healthcare services or simply providing a listening ear, Pam is always there for the members she serves. Her compassionate nature is evident in the genuine relationships she has developed over the years.

Abby Reim
Support Broker

Abby, a positive and goal-oriented individual, has always had a passion for helping others succeed in meeting their goals. Born and raised in Green Bay, she has developed a deep connection to her community and has over 10 years of experience in the field.

Abby's journey in the human services field began as a caregiver. Through her dedication and hard work, she quickly progressed into leadership and recruiting roles. Her experience as a caregiver has given her a unique perspective and understanding of the challenges that individuals face on a daily basis. This insight allows her to provide the necessary support and guidance to those in need.

Director of Operations

Community Horizons is an organization founded by Cindy, a dedicated individual with a strong passion for making a difference in people's lives. Her goal is to help individuals overcome the barriers that may prevent them from living a self-directed life. With her background in human services and human resources, she is uniquely qualified to provide additional advocacy and support to those in need.

Cindy's experience within the IRIS program has given her valuable insights into the challenges faced by individuals seeking to live independently. She understands the importance of connection and coordination in helping individuals achieve their goals. Cindy knows that it takes more than just providing resources or services; it requires a compassionate approach that acknowledges the unique circumstances and needs of each individual. She believes in treating everyone with dignity and respect, regardless of their background or situation.

One of Cindy's areas of expertise is employee relations and recruitment of caregivers. Over the course of 14 years, she has honed her skills in finding and hiring compassionate and skilled professionals who can provide the necessary care and support. She understands that finding the right caregiver can make all the difference in someone's life, and she is committed to ensuring that individuals have access to the best possible care.

Jaime Van Handel
Support Broker

Jaime’s journey into the field of advocacy within the special needs community has been largely shaped by her personal experiences as a parent. With five children, including one with complex health needs, she has had firsthand exposure to the challenges and joys that come with caring for individuals with special abilities. This has ignited a passion within her to make a difference in the lives of individuals facing similar circumstances.

Having a background in facility management and direct care has provided Jaime with the necessary skill set to excel in her role as a support broker. Her experience in facility management has equipped her with the organizational and logistical abilities needed to effectively coordinate support services for individuals with special needs. Additionally, her direct care experience has given her unique insights into the daily struggles and triumphs faced by these individuals and their families.
